Check Requests

PTA volunteers who need to request disbursement or reimbursement, please download and fill out the check request form. When the form is complete, please email it any any relevant invoices or receipts to PTA Treasurer Leilani Uttenreither at leilanipearl@gmail.com. If more than one account is being charged, please include a breakdown to help ensure accurate records.
