Check Requests

For PTA volunteers who need to request disbursement or reimbursement:

1. Click the check request form link or the blue button below.

2. When the PDF loads, download it to your computer.

3. Complete the form. Fields on the downloaded form are fillable. If more than one account is being charged, please include a breakdown to help ensure accurate records.

4. Email the completed form and any relevant invoices or receipts to PTA Treasurer Leilani Uttenreither at leilanipearl@gmail.com.
